Driver Cirsuit of HMC424LP3E

Please tell me the attached circuit.

(1) If we use TTL or CMOS input, should we input +5V or 0V into Vz?

(2) If we use TTL or CMOS input with +5V or 0V, can we get 0V or -5V output?
